Abstract Context: The number of aged people, especially with chronic or critical health conditions, has grown in recent years, demanding ever more of the current infrastructure of health services. The home health care through the use of ubiquitous computing technology can provide a solution. Objective: To design an intelligent computer system to link patients in their home and health professionals through the integration of various services related to remote monitoring of patient health. Method: analysis of the key experiences and works related to the area of ubiquitous computing in home health care, together with specialists in health, with the aim of identifying the essential requirements that must be addressed for the feasibility of a system of this class. Results: a system with a flexible architecture that integrates an infrastructure of sensors and devices, and intelligent services for (i) continuous identification of the health situation of the patient and sending alarms, (ii) targeted notifications and reminders to patients and associated with the care plan, drawn up by health professionals. A prototype implemented with a focus on data manipulation of blood pressure and heart rate is presented to demonstrate the feasibility of the proposal. Conclusion: The proposed system implements integrated services that allow continuous monitoring of the evolution of patient care in his home.
